Inspector Notes

This inspection was conducted by staff using an alternative remote protocol, including phone interview, records review, and virtual observations. An on-site inspection was not conducted at this time.

An unannounced renewal inspection was conducted at 8:57am. The head teacher
was contacted by phone to initiate the inspection. There were no children present during the inspection. The facility appeared to be clean, and classrooms were sufficiently supplied with toys and equipment for the children. Information gathered during the inspection determined there were areas of non-compliance with applicable standards or laws reviewed. Based on information submitted during the renewal application process, it was determined that 3 of 4 staff do not have a current TB screening/test completed in the last 2 years.

Evidence:

1. Staff #1 - TB screening/test was last completed 5/7/19.
2. Staff #2 - TB screening/test was last completed 8/8/19.
3. Staff #4 - TB screening/test was last completed 6/5/19.
Plan of Correction: Staff 1, 2 and 4 have been given paperwork to set up appointments and do a TB screening with the health department. All appointments will be [sic]

Based on interview conducted and records requested, there was no documentation of the most recent annual health inspection report for the facility. Evidence: The health inspection report was requested, but not received for review. The last known health inspection was completed on 3/5/19.
Plan of Correction: The annual health inspection repot has been requested from the school administration and will be posted as soon [sic]
