Inspector Notes

An unannounced monitoring inspection was initiated on 6/29/2022 and concluded on 6/29/2022. A review of staff personnel files was conducted by inspectors, Margaret Allworth, Mahrukh Aziz, Leah Pagala, Derek Acosta and Mark Davis at the Pennino SACC office. Record reviews for subs and new hires are documented with the Franconia site file.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on records review, the center did not obtain a complete sworn disclosure statement (SDS) for an employee prior to the first day of employment.
Evidence:
1. Staff # 1's first day of hire was 1/31/22.
2. Staff # 1's SDS was completed on 2/8/22.
3. Staff # 2's first day of employment was 3/28/22.
4. Staff # 2's SDS was completed on 5/18/22.
Plan of Correction: Sworn statements for all new employees will be completed prior to the first day of employment.
