During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site monitoring and self-report inspection was initiated on January 10, 2025 and completed on January 10, 2025. The self-report was received by the licensing office on December 31, 2025 relating to Child Guidance. The on-site inspection began at 3:45pm and ended at 4:30pm. The inspector reviewed compliance in the areas listed above to include standards found out of compliance during the previous inspection. There were 33 children present and 3 staff. The inspector reviewed 5 child records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Behavioral guidance should be constructive in nature, and intended to redirect children to appropriate behavior and resolve conflict.

As the group was cleaning up after snack, child A was sweeping the floor and was raising his voice, staff member 1 taunted the child with a mocking tone. When the child became agitated and held up a broom, staff 1 then took the broom and dustpan and tossed it off to the side.
Staff 1 was then witnessed picking up child A up by the arm, and pulling the child across the room.
Plan of Correction: All staff retrained on appropriate behavioral guidance of children.
