During the inspection, the inspector reviewed the areas listed above . Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 11/1/2024 and completed on 11/1/2024 in response to a complaint received by the licensing office on 10/29/2024 relating to allegations of care and protection. The inspector reviewed one staff record on-site on 11/1/2024. This inspection included document review, tour of the facility and interviews.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/22/2024. A POC submitted after this date will not appear on the public website.

When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 10/25/2024 Staff # 1 and Staff #2, did not step in to address the parent of Child #1 who, walked from her car, up to the bus and signaled Child #2 to come speak with her. Child #1?s parent was bending down to Child #2?s eye-level and speaking to the child, in response to a matter that transpired on the bus from the public school to the facility between Child #1 and Child #2. Staff #1 and Staff #2 did not intervene, and Child #2 was visibly upset and crying during the interaction with Child #1?s parent.
Plan of Correction: Staff were reminded that any behavioral incidence between children will be brought to the Directors immediate attention so that the Director can assess and decide on the proper course of action. Staff were also told that when it involves a staff member's child the staff member whose child is involved should not address other child and bring it to the Director, so she can assess and follow best course of action.
