Inspector Notes

An unannounced, on-site renewal inspection was initiated on 01/28/2025 and completed on 02/03/2025. The on-site inspection began at 9:40 am and ended at 11:20 am. The inspector reviewed compliance in the areas listed above. There were 17 children present and 2 staff. The inspector reviewed 5 children?s records on-site on 01/28/2025; 3 staff records and 6 applicant records on-site on 02/03/2025.

The inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
Staff shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children and the screening shall have been completed within the last 30 calendar days of the date of employment.

The tuberculosis screening for Staff 3 was completed 10 months prior to employment.
Plan of Correction: Will not take a TB screening older than 30 days.
