The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Child #4 did not have immunizations on file and has attended the center for over five months.
Plan of Correction: Mother brought in immunization documentation.
Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There was no physical examination in Child #4's record and the child has attended the program for over five months.

Repeat Violation

Staff are to complete the VDOE sponsored online orientation course within 90 days of hire.

Staff #1, who was working with children, has not yet completed this course which is overdue by 12 months.
Plan of Correction: Staff #1 is currently working on the course.

Any child for whom emergency medications have been prescribed shall always be in the care of a staff member who has current Medication Administration Training (MAT).

Child #6 was present at the time of inspection and had an emergency medication on site. There were no staff present with current MAT.

The center is required to obtain an annual health inspection.

The updated health inspection is overdue by two months.
Plan of Correction: There is an inspection scheduled.
Standard 8VAC20-780-330-B
Resilient surfacing shall be underneath and in the fall zones of the outside play equipment with moving parts or climbing apparatus.

There was no resilient surfacing remaining at the end of the blue slide and green slides on the playground as the mulch had degraded to soil. These slides require at least 6 inches of resilient surfacing in the fall zones.
Plan of Correction: We are getting new mulch.
