During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Violations
6Child #4 did not have immunizations on file and has attended the center for over five months.
There was no physical examination in Child #4's record and the child has attended the program for over five months.
Staff are to complete the VDOE sponsored online orientation course within 90 days of hire.
Staff #1, who was working with children, has not yet completed this course which is overdue by 12 months.
Child #6 was present at the time of inspection and had an emergency medication on site. There were no staff present with current MAT.
The updated health inspection is overdue by two months.
There was no resilient surfacing remaining at the end of the blue slide and green slides on the playground as the mulch had degraded to soil. These slides require at least 6 inches of resilient surfacing in the fall zones.
