During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 3/3/26. The on-site inspection began at 10:50am and ended at 12:00pm. The inspector reviewed compliance in the areas listed above. There were 30 children present and five staff. The inspector reviewed four children?s records and two staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/13/26. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-130-A
The center is required to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

The record for Child A, who has been enrolled for more than 20 months, did not contain documentation of immunizations.
Plan of Correction: The parent is getting a copy from the doctor's office.
Standard 8VAC20-780-500-A
Children?s hands are required to be washed with soap and running water or disposable wipes before and after eating meals or snacks.

On 3/3/26 the two-year-old and two three-year-old classes were observed transitioning directly from the classroom to the lunch tables to eat lunch, without washing their hands.
Plan of Correction: Staff will be reminded to wash hands before lunch.

The center is required to ensure that children three years of age or younger are not offered foods that are considered to be potential choking hazards.

On 3/3/26 the two three-year-old classes were observed being served whole hot dogs on buns. In order to not be considered a choking hazard, hot dogs must be cut by length and across.
Plan of Correction: Children under age 4 yrs will not be saved hot dogs on a bun.
