Inspector Notes
An unannounced renewal inspection was conducted on 5/15/2024 from 12:40m-4:150pm. The physical plant, 4 staff records, 4 agent records, 4 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed preparing for naptime and eating snack. Hand-washing procedures were also observed. Areas of non-compliance are identified in the Violation Notice.

Violations
4Evidence:
1. The most recent documentation of a tuberculosis screening in the record for Staff #2 was dated 4/17/2021.
2. The most recent documentation of a tuberculosis screening in the record for Staff #3 was dated 5/11/2022.
Evidence:
1. An evacuation drill was not practiced during the month of April 2024.
Evidence:
1. The records for Child #3 and 4 did not contain the work phone number and place of employment of each parent.
2. The records for Child #1,2,3,and 4 did not contain the address of two designated people to call in an emergency if a parent cannot be reached
3. The records for Child #1, 2, 3, and 4 did not contain documentation of viewing proof of the child's identity and age.
Evidence:
1. Child #1 and Child #3 have a diagnosed food allergy.
2. The records for Child #1 and 3 did not contain a written care plan to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
