Inspector Notes
An unannounced, on-site focused inspection was initiated and completed on 1/28/2025, as a part of probation. The Department issued a Notice of Intent to place licensee on probation on 11/27/2024 as a result of high-risk violations. The on-site inspection began at 9am and ended at 9:50am. The inspector reviewed compliance in the areas listed above. There were 19 children present and 5 staff. This inspection included tour of the facility, interviews, and observations. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
