Inspector Notes
An unannounced, on-site focused inspection was initiated and completed on 04/21/2025, as a part of probation. The Department issued a Notice of Intent to place licensee on probation on 11/27/2024 as a result of high-risk violations. The on-site inspection began at 4:00pm and ended at 4:40pm. The inspector reviewed compliance in the areas listed above. There were 24 children present and 4 staff. This inspection included tour of the facility, interviews, and observations. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
