The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. The immunizations obtained for Child #3 were dated 6 days after their first date of attendance.

Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening. The updated screening for Staff #5 was due one month ago. An updated screening has not been obtained.
Plan of Correction: Staff is in the process of scheduling an appointment.

Child day centers must maintain staff records which contain all required information. The record for Staff #3, who has been working at the center for the past 6 months, did not have documentation of 1 of 2 required references which are to be checked prior to employment.
