Inspector Notes

An unannounced monitoring inspection was conducted on May 2, 2023 from approximately 9:45 am to 10:45 am. There were 15 children in attendance and 2 staff present. 1 staff record and 3 children?s records were viewed. All areas of the center were observed including classroom, playground, and bathrooms. The children were observed playing in centers, painting, and playing games on the computer while the licensing inspector was on-site.

Violations were cited as a result of this inspection.

Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 05/12/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Based on documentation review, and interview, the center failed to provide to the licensing representative an annual fire inspection report. Evidence: The most recent fire inspection report that was posted was dated 01/11/22. When asked, staff #1, stated that she had not been provided the report. Administration acknowledged that the annual fire inspection was not provided.
Plan of Correction: Per the center "head start staff will continually follow-up with school personnel and/or Fire Marshall to receive up to date fire inspections in a timely manner."
