Inspector Notes

Two licensing inspectors conducted an unannounced complaint inspection in response to a complaint that was received by the department on January 11, 2023. The complaint was regarding supervision, injury reporting, and administration. On February 1, the inspectors were on-site from 11:40am to approximately 3:30pm. Interviews with staff were completed, observations were made at the center, and documentation was reviewed. The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint was determined to be not valid. Unrelated violations of standards were found during the course of the investigation and are identified on the violation notice.

Based on a review of documentation on 02/01/2023, the center did not ensure that written injury records contained the required information.

Evidence: Two out of three written injury records reviewed did not contain the time when parents were notified of the injuries.

The center should maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record should include the date and time of injury; name of injured child; type and circumstance of the injury; staff present and treatment; date and time when parents were notified; any future action to prevent recurrence of the injury; staff and parent signatures or two staff signatures; and documentation on how parent was notified.
Plan of Correction: Management will review with staff the requirements for written injury records to ensure all information is captured.
