Inspector Notes
An unannounced inspection was conducted on 08/28/2023 from approximately 3:05 pm to 4:25 pm in response to a complaint received on 08/25/2023 regarding allegations in the area of staff-to-children ratio.
Observation of two classrooms were conducted and interviews with the center director and three staff were conducted.
The preponderance of the evidence gathered during the investigation supports noncompliance in the area of staff-to-child ratio requirements, the complaint is determined to be valid.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not
acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Violations
1Based on observation and interviews with center staff, the center failed to ensure the 1:8 staff-to-child ratio requirement was met for children ages 2 years old.
Evidence:
1. During the inspection, the Stepping Stones room (two year olds) was observed via live video footage at approximately 3:10 pm to have 9 children with one staff.
2. During an interview with the Staff #2 and Staff #3, it was acknowledged that the center was out of ratio with 9 children being supervised by one staff. One additional staff was required.
