Inspector Notes
An unannounced complaint inspection was conducted today, September 26, 2022, in response to allegations regarding forbidden actions received by the department on August 2, 2022. The director was present during the inspection. The inspectors were on site from approximately 10:30 am until 11:55 am. Interviews with staff were conducted and observations were made. Additional interviews were conducted via phone on September 27, 2022.
The preponderance of evidence gathered during the investigation does not support the allegations of non-compliance with standards or law and therefore was not valid. Though not a part of the original complaint, a violation was found during the inspection regarding staff qualifications and training. See the violation notice for details.

1leader qualifications.
Evidence:
1. The record of staff #2, employed 09/21/2020 and identified as a program leader, did not contain documentation of additional education or 24 hours of training within six months of beginning work or within 30 days after being promoted or beginning work.
2. The record contained documentation of a high school diploma and six months of programmatic experience, however there was no documentation of 24 hours of training.
