During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated on April 13, 2026, and completed on April 13, 2026, as a part of the licensure period. The on-site inspection began at 11:00 am and ended at 1:30 pm. The inspector reviewed compliance in the areas listed above. There were 76 children present and 12 staff. The inspector reviewed seven children?s records and eight staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on April 20, 2026. A POC submitted after this date will not appear on the public website.

The center is required to have employees undergo a background check every five years. 1. The most recent central registry background check for staff #1 was dated over five years ago. An updated central registry was overdue by eight days.

Standard 8VAC20-780-160-C
At least every two years from the date of the initial screening or testing staff members and individuals from independent contractors shall obtain and submit the results of a follow-up tuberculosis screening. The record of staff #2 contained documentation of a tuberculosis screening that expired one month and 26 days ago. An updated tuberculosis screening has not been conducted.

Any child for whom emergency medications (such as albuterol, glucagon, and epinephrine auto injector) have been prescribed shall always be in the care of a staff member or independent contractor who meets the requirements of Medication Administration Training (MAT). Child #8, child #9, child #10, and child #11 have emergency medications on site that the center has agreed to administer if needed. The staff present during the inspection were not currently certified in Medication Administration Training (MAT).

All staff who work directly with children shall have within 90 days of the date of employment current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care. The training shall include an in-person competency demonstration. Staff #8, employed for five months, did not have current CPR certification.
