Inspector Notes

An unannounced monitoring inspection was conducted on 11/21/2023 from 9:40am to 10:40am. There were 6 children ages 2 years old to age 7 years old (10 points) with the Provider. The programming area, attendance, menus, Provider, Assistant and Household member records, 6 children?s records, emergency drills, emergency supplies, and policies were reviewed. The outdoor play area was not inspected due to weather. There were no medications on site. Children were observed participating in breakfast, watching tv and free play. Handwashing procedures were also observed. The Provider had current certification in MAT, CPR and First Aid training. Areas of non-compliance are identified in the Violation Notice.

Based on review of 3 Household member records, the provider did not obtain a sworn disclosure every five years from the date of the last sworn disclosure.

Evidence: The record for Household member #2 did not contain documentation that a sworn disclosure had been completed every five years from the date of the last sworn disclosure. The last sworn disclosure was completed 6/28/18.
Plan of Correction: A new updated sworn disclosure has been signed.
