During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated and completed on 6/24/2025, as part of an Intensive Plan of Correction (IPOC). The Department requested that the provider complete an IPOC as result of the last inspection related to administration, physical plant, and special care provisions and emergencies.

The on-site inspection began at 10:45 a.m. and ended at 12:30 p.m. The inspector reviewed compliance in the areas listed above. There were 43 children present and seven staff. The inspector reviewed six children?s records and three staff records on site. This inspection included document review (i.e. daily schedule, menu, license, previous inspection, attendance records, injury reports, emergency drill logs, and medication authorization forms), tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 7/02/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-40-E
(Repeat Violation) The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards and the center's own policies and procedures that are required by these standards. The center did not comply with its policies regarding preventing the use of outdated medication. The medication for child #6 expired 5/2025. The medication was administered twice a day on 6/02/25-6/06/25, 6/09/25-6/13/25 and on 6/23/25.

Standard 8VAC20-780-510-B
(Repeat Violation) Prescription and nonprescription medication shall be given to a child only with written authorization from the parent. The authorization for the medication for Child #6 expired on 6/14/25. Child #6 was administered medication twice on 6/23/25.

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include staff and parent signatures or two staff signatures. An injury report dated 5/02/25 did not contain staff or parent signatures.
