Inspector Notes

An annual monitoring inspection was initiated on 01/31/2023 and concluded on 01/31/2023. The inspector was on site at the center from 9:55am to 11:05am. There were 16 children in care with 4 staff supervising. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. The center?s playground and all classrooms were inspected today. Five children?s records and six employee records were reviewed during this inspection. Information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and a violation was documented on the violation notice issued to the vendor.

Based on record review, the facility failed to obtain repeat central registry results for four of six staff within five years since the date of the last central registry result.
Evidence:
1. The record of staff #1 (employment date: 9/1993) had documentation of central registry results dated 3/24/16.
2. The record of staff #3 (employment date: 9/2013) had documentation of central registry results dated 5/25/16.
3. The record of staff #4 (employment date: 8/2017) had documentation of central registry results dated 10/10/17.
4. The record of staff #6 (employment date: 1/2017) had documentation of central registry results dated 2/16/17.
Plan of Correction: Applications for updating Central Registry requests were distributed 1/30/23. All information will be uploaded to the CCAPS site as soon as possible. Results will be emailed to [the inspector] as soon as they are available.
