Inspector Notes

An unannounced inspection was initiated on September 22, 2025 and completed on September 22, 2025, 2025, in response to complaint received by the licensing office September 19, 2025 relating to allegations in the areas of supervision and abuse reporting. The inspector reviewed one child?s records and one staff record on-site on September 22, 2025. This inspection included document review, a tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on January 12, 2025. A POC submitted after this date will not appear on the public website.

The center is to establish and implement procedures for appropriate supervision of children in care.

The center's procedures for appropriate supervision include a management of behavior policy. The policy states that there shall be no physical punishment. On 09/12/2025, Staff #1 dragged child by their shirt to another area of the room. Staff #1 was sitting in a chair and used her foot two times to push child #1 away from her. Staff #1 grabbed child #1 by both arms and forcefully places child #1 in a chair.
Plan of Correction: The program will regularly review security camera footage to ensure appropriate supervision and staff interactions in the classroom. The program has hired an additional staff member, separate from the assigned support staff. All staff, including new hires, receive ongoing training on supervision, appropriate guidance, and classroom management. The Director is actively involved in classroom activities, monitors for any out-of-the-ordinary situations, and conducts regular check-ins with teaching staff. The Director will continuously monitor the effectiveness of these preventative measures through regular classroom observations, review of security camera footage, and ongoing communication with staff. Any concerns or unusual situations will be addressed immediately. The program will maintain appropriate staffing levels, provide ongoing training, and make adjustments as needed to ensure continued compliance and the safety of all children.The Director is responsible for overseeing implementation of all preventative measures, including reviewing security camera footage, conducting classroom observations, and ensuring appropriate staffing and supervision. The Lead Teacher is responsible for daily classroom supervision and immediately reporting concerns to the Director. Support Staff assigned to Child #1 are responsible for providing continuous supervision and support and communicating any concerns to the Lead Teacher and Director. All teaching staff are responsible for following program policies, participating in required training, and maintaining proper supervision at all times.
