Inspector Notes

An unannounced renewal inspection was conducted on November 13, 2023 at 9:00 am until 10:34 am and completed on November 16, 2023. There were 13 children present with 3 staff supervision, and 1 administrative staff on site. The inspector completed observations of the children in care participating in circle time, engaged in centers, working on parts of the pumpkin activity with staff, completing bathroom breaks/handwashing, and active engaged in outdoor playground activity. A total of 5 child records and 3 staff records were reviewed. The Inspector reviewed indoor areas, staffing and supervision, activities, equipment, emergency evacuation, and shelter-in-place drill documentation, emergency procedures and required posting. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program. Based on observation and photographs, the center did not ensure all area and equipment of the center inside and outside are maintained in a clean, safe and operable condition.

Evidence:
1. In the toddler bathroom (231), the tile was observed cracked with the concrete wall exposed next to the toilet paper dispenser.
2. Photograph of the condition of the bathroom area taken during the inspection.
3. There were multiple rusting areas on the metal toilet paper dispenser in use by children during the inspection.

Based on observation and photograph, the center did not ensure the temperature shall be maintained no lower than 68 F in inside areas occupied by children.

Evidence: In 1 out 3 classrooms in use by the children in care, the thermostat temperature was 64 F during the inspection.
