Inspector Notes

An unannounced renewal inspection was conducted on-site February 27, 2024 from 10:26amuntil 1:40pm. The director was available during the inspection. There were 78 children present, ranging in ages from 6 weeks to 5 years, with 16 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 7 child records and 7 staff records were reviewed. Safe sleep guidance was provided to the center.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations

1
Standard 8VAC20-780-370-1
Based on observation and interview, the center failed to ensure when an infant is placed in his crib, he shall be placed on his back (supine).
Evidence: Infant #1 (age 6 weeks) was observed to be placed in the crib on the infant's left side to sleep. Administration acknowledged the infant was placed on his side, and not on his back (supine).
Plan of Correction: Bright Beginnings has implemented corrective measures to ensure compliance when staff place infants in their cribs they are always placed on their backs. The way this noncompliance will be mitigated in the future will be by ensuring all staff are retrained on the expectations of standards. Directors will be responsible for retraining staff and ensuring they are following the standards expected of them.
