Inspector Notes

A Religiously Exempt Child Day Center Code Compliance inspection was completed on July 11, 2022 from 11:26am to approximately 1:10pm. There were 35 children present and seven staff members. A sample size of five children?s records were reviewed and five staff members. Child and staff interactions were observed during the entire time of the inspection. First Aid Kits and Emergency preparedness drill logs were observed. Inspector was able to view handwashing, free play, and naptime. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program. The violations are listed on the violation notice issued to the center and were reviewed with the site owner at the exit interview.

Based on record review and interview, it was determined that any individual required to undergo a background check in accordance with subsection A shall submit to fingerprinting.
Evidence:
1. Staff #2 (date of hire: 6/27/2022) does not have their fingerprints completed for their background check.
2. Staff #4 (date of hire: 06/21/2022) does not have their fingerprints completed for their background check.
3. Staff #1 confirmed that both Staff #2 and Staff #4 did not have fingerprints completed to gather the results of their background check.
4. Staff #1 confirmed that both Staff #2 and Staff #4 are working with children in care at the center.
Plan of Correction: The center responded with the following: Staff #1 will have Staff #2 and Staff #4 complete their fingerprints completed for their background checks and have the results placed in their record.

Based on record review and interview, it was determined that the center did not ensure that prior to the first day of employment that the signed sworn disclosure statement by staff be in the record.
Evidence:
1. Staff #2 (date of hire: 06/27/22) has a sworn statement signed on 7/07/2022, which is signed after their first day of employment.
2. Staff #4 (date of hire: 06/21/2022) has a sworn statement signed on 06/24/2022, which is signed after their first day of employment.
3. Staff #1 confirmed that both Staff #2 and Staff #4 did not have their sworn statement signed prior to the first day of employment.
Plan of Correction: The center responded with the following: Staff #1 will ensure that the sworn statement will be signed prior to their first date of employment.
