Inspector Notes

A complaint inspection was conducted on August 31, 2023, from 1:50pm to approximately 2:45pm, in response to a complaint that was received by the department on August 28, 2023, regarding allegations in the care of the children at the center. Staff were interviewed, documentation was reviewed, classroom observations were completed, the center?s policy and procedures were reviewed, and a tour of the facility was conducted. The evidence gathered during the investigation determined that it is not valid for the concerns of the care of children in the care however there is a valid concern with regards to the reporting process. A violation was issued to the center and the violation was discussed at the exit interview with the site director.

Based on record review and interview, it was determined that the center failed to inform the department's representative as soon as practicable, but not more than two business days, of any injury to a child while under the vendor's supervision when a referral is made for treatment from a medical professional.
Evidence:
1. On August 17, 2023, Child #1 tripped and fell in the classroom causing an injury to their arm. Staff #2, the child?s teacher, immediately informed the director and the parent was called. The parents transported the child to the hospital. Staff #2 completed a written injury report.
2. Staff #1, the director, confirmed that they did not inform a Department of Education (DOE) representative within two business days of the incident. The Injury of a Child Requiring Outside Medical Treatment report to DOE was filled out on August 30, 2023.
Plan of Correction: The center responded with the following: The Injury of a Child Requiring Outside Medical Treatment or Death form was filled out during the time of the inspection with DOE. Moving forward, the center will ensure to inform a DOE representative and fill out the form in a timely manner.
