8VAC20-790 Subsidy Program Vendor Requirements for Child Day Centers
Inspector Notes
A subsidy health and safety inspection (SHSI) was completed on March 20, 2024 from 9:40am to approximately 11:35a. A Religiously Exempt Child Day Center Code Compliance inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in code compliance were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. The violations are listed on the violation notice issued to the center and were reviewed with the site director at the exit interview.
Violations
2which shall be made accessible to the department?s representative upon request. The child's record shall include telephone number for each custodial parent's place of
employment and the address of at least one person designated in case of an emergency and the parent cannot be reached.
Evidence:
1. Child #1 (date of enrollment 9/26/22) did not have the work phone for their father.
2. Child #2 (date of enrollment 3/12/24) did not have addresses for their emergency contact
3. Staff #1 confirmed that a work phone number and address for an emergency contact was not in the records.
Evidence:
1.In the children?s bathroom, there were rusted screws, approximately 1 1/2 inches in length, protruding from the base of the commode, which poses as a tripping and injury hazard.
2.The carpet in the large common area was fraying, which poses as a tripping hazard.
3.In the toddler room, there was a protruding nail on the bulletin board, which is at the children?s height. This poses as a snagging and injury hazard to the children in care.
4.In the infant play area, the wire to the radio was accessible to children
