During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated and completed on 9/22/25 in response to a complaint received by the licensing office on 7/23/25 relating to allegations of forbidden actions. The inspector reviewed five children?s records and two staff records on-site. This inspection included document review and interviews. The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/5/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-790-550-2-f
The vendor shall obtain documentation of subsequent background checks conducted every five years.
The record for Staff #2 (employed for 11 years) did not contain an up to date sworn statement or criminal history background check, which are required every five years.
Plan of Correction: To establish a process for regularly updating teacher files and ensuring compliance with background check requirements. Confirm that background checks are conducted every five years for the teacher. By implementing this correction plan, we ensure that all files remain current.
Standard 8VAC20-790-560-A
Staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children.
Staff #1 did not submit documentation of a TB screening at the time of employment and prior to coming into contact with children, which is required.
Plan of Correction: To ensure that all staff members submit their TB documents in a timely manner and maintain up-to-date health paperwork. The staff member is currently up-to-date with their TB form. Ensuring ongoing health and safety for the entire team and the children in our care.

Behavioral guidance shall be constructive in nature and age and stage appropriate and
shall be intended to redirect children to appropriate behavior.
On 7/23/25, Staff #1 did not use behavior guidance that was appropriate and intended to direct a preschool age child's behavior during rest time.
Plan of Correction: Techniques for redirecting children who exhibit behavioral problems during nap time, ensuring a calm and conductive environment for all students. Training program focused on redirection and classroom management strategies specifically for nap time.

The following actions or threats thereof are forbidden: verbal remarks that are demeaning to the child.
On 7/23/25, Staff #1 verbally threatened a preschool child during nap time in order to get the child to lay down.
Plan of Correction: To ensure that all teachers communicate in a respectful manner and do not repeat comments referring what the parent said to get the child to lay down. Organize mandatory training sessions focused on effective communication strategies with children. Having positive reinforcement and age appropriate language use.
