8VAC20-790 Subsidy Program Vendor Requirements for Child Day Centers

Inspector Notes

A subsidy health and safety inspection (SHSI) was conducted on 7/20/22 from 1:15pm until 2:10pm. There were 7 children present with 2 staff. A third staff member arrived during the inspection. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in code compliance were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law. Violations are listed on the violation notice issued to the center and were reviewed with the site owner at the exit interview.

Based on record review and interview, the vendor did not ensure that children's records contain all the required information.

Evidence:
1. The record for child 1 (date of enrollment: 7/8/19) does not contain the name address and telephone number of at least one person designated by the parent to contact in case of an emergency and the parent cannot be reached.
2. The center director confirmed that the record for child 1 is lacking the required emergency contact information.
Plan of Correction: The vendor responded with the following: The vendor will contact child 1's parent and obtain the emergency contact information and it will be documented in the child's record.

Based on record review and interview, the vendor did not ensure that all staff shall have current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.

Evidence:
1. The record for staff 1 contains documentation of CPR certification dated February 2020. Staff 1 was due for CPR certification renewal in February 2022.
2. The center director confirmed that CPR certification for staff 1 has expired.
Plan of Correction: The vendor responded with the following: Staff 1 will obtain CPR recertification as soon as possible.

Based on record review and interview, the vendor did not ensure that all staff have current certification in first aid appropriate to the ages of children in care.

Evidence:
1. The record for staff 1 contains documentation of first aid certification dated February 2020. Staff 1 was due for first aid recertification in February 2022.
2. The center director confirmed that staff 1's first aid certification is expired.
