An unannounced complaint investigation was initiated on 3/20/23 and was concluded on 4/21/2023 in response to a complaint received by the licensing office on 03/01/2023 regarding staffing and supervision. Additional complaints were received on 3/27/23 and 4/4/23 regarding the same allegations as well as building maintenance. Observation, record reviews, and interviews were conducted relating to the allegations on 3/22/23, 3/28/23, and 4/4/23. The information gathered during the investigation does not support the allegations, so the complaint is determined to be " not valid".
