Inspector Notes

An unannounced inspection was initiated on October 10, 2024 and completed on October 18, 2024 in response to a complaint received by the licensing office on September 9, 2024 relating to allegations of background checks, staffing and supervision and physical plant. The inspector reviewed 4 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews and observations. The preponderance of evidence gathered during the investigation supports the allegation of background checks, therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/31/2024 A POC submitted after this date will not appear on the public website.

Providers must obtain a completed national criminal background check prior to the employee's first day of employment.
Staff #1, Staff #2, Staff #3 and Staff #4 who had been employed for over a year, did not have a completed national criminal background check.
Plan of Correction: Center made contact with OBI to update contact information of new director & administrator. All background checks will be sent to current email address
and director. All staff have completed background checks

The center must request a search of the central registry prior to the employee's first day of employment.
Staff #2, who had been employed for 8 months, did not have a central registry search requested.
Plan of Correction: Printed & Digital reminders of renewal dates will be posted & entered to ensure
compliance of central registry search to to be completed every five years. Searh was requested for staff member.
