A subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection. Subsidy standards that are not covered in code were reviewed during this SHSI. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

The child?s record shall include the following: The address of at least one person designated by the parent to contact in case of an emergency and the parent cannot be reached. The record for child #1, child #2, child #3, child #4 and child #5 did not have an address listed for their emergency contacts.
Plan of Correction: The emergency contact information for child #1, #2, #3, #4, and #5 has been updated to include the address of at least one designated emergency contact. Going forward, procedures have been revised to ensure emergency contact addresses are obtained and verified during enrollment.

Staff who work directly with children shall annually attend at least 16 hours of training to include the department's health and safety update course. The record for staff #1 did not have documentation of a completed 3 hour preservice annual training.
Plan of Correction: Staff #1 completed the required 3-hour preservice training. Documentation is now on file. A tracking system has been implemented to monitor training compliance. Staff will complete required training annually, with quarterly reviews by the director to ensure timely completion.
