Inspector Notes

An unannounced Renewal Inspection took place on 09/11/2023 between the hours of approximately 11:30 a.m. and 1:10 p.m. There were 4 classrooms observed with a total of 19 children with 4 staff within the supervision guidelines. The children were observed engaging in free play, identifying animals and other organized activities. Positive interaction between staff and children were observed. A complete inspection of the physical plant, children and staff records, fire drill log, medication and emergency procedures were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-780-140-A
Based on record review, the center failed to obtain physical examinations before the child?s attendance or within 30 days after the first day of attendance for 2 of 5 children records reviewed.

Evidence:
1. Child #4 (start date: 07/05/2022) did not have a physical examination on file.
2. Child #5 (start date: 05/01/2023) did not have a physical examination on file.
Plan of Correction: The center has returned incomplete physical forms to both child's parents and have asked to return the form once signed by their child's physician.
Standard 8VAC20-780-260-B
Based on record review and staff interview, the center failed to obtain annual approval from the health department for meeting requirements for water supply and sewage disposal system.

Evidence: Staff #1 confirmed that an environmental sanitation inspection had not been conducted.
Plan of Correction: The center contacted the health department to receive more information regarding this inspection. The center submitted an application to have a CORE/Health Sanitation Inspection completed by the health department.
Standard 8VAC20-780-40-K
Based on record review and staff interview, the center failed to develop written procedures for prevention of shaken baby syndrome or abusive head trauma, safe sleeping practices and sudden infant death syndrome awareness.

Evidence: Staff #1 confirmed that the center did not have written procedures for prevention of shaken baby syndrome or abusive head trauma, safe sleeping practices and sudden infant death syndrome awareness.
Plan of Correction: The center will put together a binder which contains written procedures for prevention of shaken baby syndrome, abusive head trauma, safe sleeping practices and sudden infant death syndrome awareness which will be kept in the office. The center will also include a copy of said procedures in each classroom's emergency binders to provide easy access at all times for staff.

Based on record review and staff interview, the center failed to develop written playground safety procedures.

Evidence: Staff #1 confirmed that the center did not have written playground safety procedures.
Plan of Correction: The center will develop written playground safety procedures based off the verbal version told to all staff members upon hire. A copy will be put in the emergency binders of each class.
