Inspector Notes
An unannounced monitoring inspection was conducted with the director on December 18, 2023, beginning at 12:20pm and ending at 1:45pm. There were 27 children, ages 14 months through 5 years old, in the care of 5 direct care staff. The children were observed sleep or laying on resting mats during nap time. 5 children?s records were reviewed and in compliance. Four staff records were reviewed. The first aid kit, flashlight, and battery-operated radio were observed and in compliance. Three medications were reviewed and in compliance. The menu, daily attendance, and emergency drill log were reviewed. The most recent evacuation drill was documented as 12/04/2023. The most recent shelter-in-place drill was documented as 8/3/2023. The most recent lockdown drill was documented as 8/8/2023. The most recent health inspection was dated 3/9/2023. The most recent fire inspection was dated 3/16/2023.

Standard 8VAC20-780-160-A
Repeat Violation - Based on a review of four staff records, not all of the staff had documentation of a negative tuberculosis at the time of employment and prior to coming into contact with children. Evidence: Staff 1, start date 8/28/2023, had documentation of a tb screening dated 8/29/2023.
Plan of Correction: Moving forward, the director will ensure that new staff have documentation of a negative screening prior to having contact with the children.
Standard 8VAC20-780-270-A
Based on observation, not all of the areas and equipment inside and outside of the center were maintained in a safe and operable condition. Evidence: In the 2/3's classroom, the doors to the under counter cabinet did not close all the way.

Standard 8VAC20-780-500-A
Based on observation, not all staff washed their hands with soap and running water before and after a diaper change. After changing a child's diaper, the staff member did not wash their hands.
Plan of Correction: The staff member was reminded to wash their hands after completing the diaper change.
Standard 8VAC20-780-500-B
Based on observation, the diapering surface was not cleaned with soap and at least room temperature water and sanitized after each use. Evidence: The diapering surface was not sanitized after the observed diapering.
Plan of Correction: The diapering surface was cleaning with a cleaning solution. The staff will be retrained on the two step process of cleaning and sanitizing the diapering surface after use.
