Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9/10/2024 and completed on 9/10/2024. The on-site inspection began at 11:12 am and ended at 12:30 pm. The inspector reviewed compliance in the areas listed above. CDC: There were 33 children present and 6 staff. The inspector reviewed 5 children?s records and 3 staff records on-site on 9/10/2024. This inspection included document review, tour of the facility, interviews, observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/26/2024. A POC submitted after this date will not appear on the public website.

REPEAT VIOLATION: Based on observation, not all areas and equipment inside and outside of the center were maintained in a clean, safe, and operable way. Evidence: The changing pad in the infant classroom had small particles and debris on it. The wooden gate on the toddler playground had three rusty nails protruding out of the fence. A full panel of the wooden gate was leaning towards the wooded area behind the center. There was a gap between the bent section of the wooden gate and chain link fence creating an entanglement hazards.
Plan of Correction: A contractor has been contacted to replace the wooden fence with a new chain-link fence. The work is scheduled to begin at the end of September.
