A Subsidy Health and Safety inspection was conducted on May 9, 2023 to review supplemental health and safety requirements.. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Areas of non compliance may be found on the violation notice.

REPEAT VIOLATION
Based on record review the center failed to have one staff member complete the departs annual health and safety course.
Evidence:
1. Five staff records were reviewed.
2. Staff #3 did not have documentation of completing the departments annual health and safety training.
3. Staff #3 last documented training was dated 03/14/2021.
4. The director confirmed there was no documentation of Staff #3 completing the required course.
