Inspector Notes

An unannounced monitoring inspection was conducted on December 5, 2023, from 11:15 a.m. through 12:45 p.m. Upon arrival, there were 24 children, ages 7 months through 54 in the care of 4 staff.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 children and 7 staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review the center failed to obtain a tuberculosis screening for one new staff member within 30 days of date of employments.

Evidence:
1. Staff #3's date of hire was documented on 06/24/2023. Tuberculosis screening was dated 05/10/2022.2.
2. The director confirmed a tuberculosis screening for Staff #3 was not dated 30 days within their employment.
