During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on November 12, 2024, as a part of the licensure period. The on-site inspection began at 10:00 a.m. and ended at 12:00 p.m. The inspector reviewed compliance in the areas listed above. There were 29 children present and 6 staff. The inspector reviewed 5 children?s and six staff records. This inspection included document review tour of the facility, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on December 3, 2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. One child has been in care for two months without documentation of obtaining immunizations.
Plan of Correction: Immunizations were obtained 11/13/24.
Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. Disinfectant wipes with the warning label, "Keep out of reach of children," were unlocked in the two year old classroom.
Plan of Correction: Lock box was purchased for hazardous substances.
Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance. A seven month old child was observed with a string bracelet with small beads on their wrist. The staff said they do not remove the bracelet when the child is sleeping. The American Academy of Pediatrics state "Teething necklaces, bracelets and other jewelry are unsafe and should not be worn by infants or children, according to the Food and Drug Administration (FDA)."
Plan of Correction: Parent was informed at pickup, 11/12/24, that the infant can not wear jewelry to daycare.
Standard 8VAC20-780-430-B
Materials and equipment available shall be age and stage appropriate. Hot Wheels toy cars were in the two-year old classroom. The manufacture warning label states "Not suitable for children under 36 months." Children were observed playing with the cars.
Plan of Correction: Cars were removed immediately from the classroom.
Standard 8VAC20-780-500-B
A nonabsorbent surface for diapering or changing shall be used. The diapering pad had a small tear making it unabsorbent in the toddler classroom.
Plan of Correction: A new pad was ordered an will arrive 11/18/24.
Standard 8VAC20-780-560-G
When food is brought from home the food container shall be dated and labeled in a way to identify the owner. Three lunchboxes in the preschool room were not labeled with the child's name.
Plan of Correction: Lunch box was labeled that day.
