8VAC20-780 Staffing and Supervision.

During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, inspection was initiated and completed on February 3, 2025, with a local agency in response to a complaint received by the licensing office on January 30, relating to allegations of standard 340-A. There were twenty-four children present and six staff. This inspection included document review, tour of the facility, interviews and observations.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on February 20. A POC submitted after this date will not appear on the public website.

When staff are supervising children, they shall ensure their care, protection and guidance. On January 29, 2025, a visitor entered the facility and observed 10 children, ages 2-3, during nap time. Staff #1, responsible for supervising the children, was sitting in a chair with her head down and did not acknowledge the visitor. The visitor walked around the room and into an adjacent room with no adult acknowledging his presence. The visitor exited the facility and then entered again to find Staff #1 still sitting in the chair with her head down. Staff #1 did not acknowledge the visitor until he walked up to her and began speaking.
