Inspector Notes

An unannounced monitoring inspection was initiated on 07/17/2025 and completed on 08/04/2025 in response to a self-report received by the licensing office on 07/09/2025 relating to supervision. The inspector reviewed 2 children?s records on-site and 2 staff records electronically on 08/04/2025. This inspection included document review, tour of the facility, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The vendor shall obtain documentation that each child has received the
immunizations required by the State Board of Health before the child can attend
the center.
There were no documentation of immunizations for Child 1 and Child 2.
Plan of Correction: Provider will obtain before attendance.
