Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated and concluded on August 18, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 43 children present, ranging in ages from 6 months to 5 years old, with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-185-130-A
Based on record review and interview, the center did obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence:
1. Child #1's(Start date-01/25/21) record did not contain immunization records.
2. Staff #4 confirmed that the immunization records were not obtained from the parents.
Plan of Correction: The center will contact the parents and obtain a copy of the immunization records.
Standard 22VAC40-185-140-A
Based on record review and interview, each child did not have a physical examination by or under the direction of a physician.
Evidence:
1. Child #1's(Start date 01/25/21) record did not contain a copy of a physical examination.
2. Staff #4 confirmed that a copy of a physical examination was not obtained from the parents.
Plan of Correction: The center will contact the parents and obtain a copy of the most recent physical examination.
Standard 22VAC40-185-550-B
Based on record review, the emergency preparedness plan was not updated annually.
Evidence:
1. The Emergency preparedness Plan was not updated annually. The last time that the plan was updated was 11/1/2017.
Plan of Correction: The center will review the plan and update for this school year.
Standard 22VAC40-185-570-B
Based on observation, bottle fed infants who cannot hold their bottles were not held when fed.
Evidence:
1. Staff #1 was observed feeding Child #4 a bottle while they were sitting in a bouncy chair.
Plan of Correction: The child was removed from the bouncy chair immediately and held while being fed a bottle during the time of inspection.

Based on record review, children's records did not contain required information.
Evidence:
1. Child #2's record (Start date-6/14/21) did not have documentation of proof of birth.
2. Child #3's record (Start date 2/22/21) did not have documentation of proof of birth and a second emergency contact.
Plan of Correction: The center will obtain information from parents and update the file.

Based on observation, the findings of the most recent inspection of the facility was not posted.
Evidence:
1. The current Inspection Notice was not posted. The last inspection notice posted was dated 10/22/2019.
Plan of Correction: The center will print current inspection notice and post it.
