Inspector Notes

An unannounced monitoring inspection was conducted on January 29, 2024 between the hours of 10:15am-12:00pm. There were a total of 39 children with 13 supervising staff members present. Children were observed during morning snack time, handwashing, toy play, engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard 22.1-289.035-A
Based on record review of four staff records, the center did not update criminal history record reports every five years.
Evidence:
1.Staff #1's record (Date of hire-01/27/2009) did not contain results from a repeat criminal history background check every five years. Documentation of the criminal history results was dated 11/01/2018.
Plan of Correction: The staff member will make an appointment with Fieldprint to have their fingerprints taken to obtain an updated criminal history record. Moving forward, management will conduct a file check to ensure that criminal history checks are conducted every five years.
Standard 8VAC20-780-160-C
Based on record review, subsequent tuberculosis(TB) screenings were not obtained for staff members every two years.
Evidence:
1. Staff #'1's record (Date of hire-01/27/2009) did not contain a subsequent TB screening. Documentation of the last TB screening was dated 10/14/2021.
2. Staff #4's record (Date of hire-12/13/2021) did not contain a subsequent TB screening. Documentation of the last TB screening was dated 12/02/2021.
Plan of Correction: Staff members will make an appointment to have the TB screenings conducted. Management will ensure to perform a file check to ensure that all tb screenings are conducted every two years.
Standard 8VAC20-780-280-B
Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the Pre-K classroom, a bottle of deodorizer and disinfectant were located on a high shelf in the bathroom. These items were not kept in a locked place using a safe locking method.
Plan of Correction: The deodorizer and disinfectant were locked up at the time of the inspection. Staff will ensure to lock up all hazardous substances.
