Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on July 31, 2025 as a part of the licensure period. The on-site inspection began at 9:10am and ended at 10:40am. The inspector reviewed compliance in the areas listed above. There were 32 children present and 11 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Child #4 did not have documentation of immunizations required before the child attended the center. The child has been enrolled for eight days.
Plan of Correction: The parent of Child #4 has been contacted to obtain current immunizations. Moving forward, the center will ensure to obtain immunizations before children's first day of attendance.
Standard 8VAC20-780-160-C
At least every two years from the date of the first initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening.

Staff #4 did not have documentation of a subsequent tuberculosis screening. Staff #4's record contained an expired tuberculosis screening dated over two years ago.
Plan of Correction: Staff #4 will get a TB screening today and document the record.
