Inspector Notes

An unannounced monitoring visit and facility tour was conducted on 1/25/2023. At the time of inspectors' arrival there were 18 infant to preschool aged children in care with 4 staff members. Children were observed interacting with staff, nap, and hand washing. A sample of 5 children's records and 4 staff records were reviewed. Assistant Director reported that there have been no injuries and medications are not being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Based on record review and interview it was determined the center did not ensure that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence:
1. There were no immunizations in the record for child #2(enrolled 11/2/22).
2. Staff confirmed this documentation was not available during this inspection.
Plan of Correction: Staff will request immunization records from the parents.
Standard 8VAC20-780-130-E
Based on record review and interview it was determined the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1. There were no updated immunizations in the record for child #1, #3, and 5. The documentation available was dated 3/10/22, 7/22/19, and 1/13/33 respectively, the children were all under two years at this time.
2. Staff confirmed this documentation was not available during this inspection.
Plan of Correction: Staff will request immunization records from the parents.
Standard 8VAC20-780-140-A
Based on record review and interview it was determined the center did not ensure that each child has received a physical examination by or under the direction of a physician within the required time frame.
Evidence:
1. There was no physical exam in the record for child #2(enrolled 11/2/22).
2. Staff confirmed this documentation was not available during this inspection.
Plan of Correction: Staff will request documentation of physical exam from the parents.
Standard 8VAC20-780-270-A
Based on observation it was determined the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.
Evidence:
1. There were kitchen knives in an unlocked drawer in the kitchen.
2. Staff confirmed that they needed to replace the gate to the kitchen.
Plan of Correction: Staff will prevent access to sharp kitchen utensils.
