Inspector Notes

An unannounced monitoring visit and facility tour was conducted on 12/07/2023. At the time of inspectors' arrival there were 19 infant to preschool aged children in care with 3 staff members and 1 administrators. Children were observed interacting with staff, nap, snack, and hand washing. A sample of 5 children's records and 3 staff records were reviewed. Director reported that there have been no injuries and medications are not being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Based on record review and interview it was determined the center did not ensure each child had a physical examination by or under the direction of a physician.
Evidence:
1. The record for child #1(enrolled 6/3/22) did not contain documentation of a physical exam.
2. Staff confirmed that the record for child #1 did not contain documentation of a physical exam.
Plan of Correction: Staff will request documentation from parent.
Standard 8VAC20-780-260-A
Based on record review and interview it was determined the center could not provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction.
Evidence:
1. The fire inspection was dated 2/2/2022.
2. Staff confirmed that they did not have a more recent fire inspection.
Plan of Correction: Center will request an updated fire inspection.
