Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

The records for Staff #3 and Staff #4 showed that a five year repeat central registry search was completed three months after expiration.

The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
The record for Child #1and Child #2 showed that the children's immunizations were not received prior to the child attending the center.

Each child shall have a physical examination by or under the direction of a physician.
The record for Child #2 (enrolled for seven months) did not contain documentation of a physical examination.

The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.
The record for Staff #1 (employed for five months) did not contain documentation to show that The Virginia Department of Education-sponsored orientation course was completed.

Staff shall complete annually a minimum of 16 hours of training appropriate to the age of children in care.
The records for Staff #2, Staff #3, and Staff #5 did not contain documentation that 16 hours of annual training was completed in 2025.

Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
In a pre-kindergarten classroom, disinfectant wipes and hand sanitizer were stored on a shelf inaccessible to children. In a play room, hand sanitizer was stored on a desk accessible to children. In a school age classroom, hand sanitizer was stored on a high shelf inaccessible to children. All of the mentioned substances were stored unlocked, but are required to be locked.

One staff member shall not supervise more than one room or area during rest time.
At the inspector's arrival, one staff member was supervising two rooms during rest time.

Repeat Violation
Staff records shall be kept for each staff person and contain all the required information.
The record for Staff #1 did not contain documentation that two or more references as to character and reputation as well as competency were checked before employment. One reference for the staff member was checked.

Staff who work directly with children shall annually complete the department's health and safety update course.
Staff #2 did not complete the department's health and safety update course in 2025, which is required annually.
