Inspector Notes

A non-mandated inspection was initiated on February 11, 2022 and concluded on February 22, 2022. A self-reported incident was received in the Office of Child Care Health and Safety (Valley Office), on February 11, 2022 regarding behavior management and forbidden action. Interviews were conducted on 02/14/2022 in conjunction with a local agency. One child record, six interviews, attendance records, and observations of the classroom were conducted.

Information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the program.

Standard 63.2-1509-A
Based on interviews, staff failed to report suspected child abuse and neglect in a timely manner.

Evidence:
1. Staff #2 stated in an interview on 02/14/2022, that Child #1 had told her on 02/04/2022 that she was scared of Staff #3, and that Staff #3 had pinched her.
2. Staff #2 did not check Child #1 for any marks, nor did Staff #2 report the incident to administration.
3. Administration was made aware of the alleged abuse on 02/07/2022 by the parent.
4. Administration reported the incident to Child Protective Services and Office of Child Care Health and Safety on 02/09/2022. Staff #3 was put on administrative leave until the investigation was completed by both entities.
Plan of Correction: Disciplinary action was taken on the staff for not reporting the incident. All staff will attend a Mandated Report training for Child Abuse and Neglect on March 1, 2022. Staff will continue to actively work on Conscious Discipline training and classroom management training.
