Inspector Notes

A non-mandated focused inspection was completed on February 22, 2024 from 11:00 A.M.-12:45 P.M. There were 44 children present, ranging in the ages of four months to five years of age, with ten staff supervising. The inspector reviewed compliance in the areas of administration, staff qualifications and training, physical plant, staff and supervision, programs, special services and background checks. A total of three staff members records were reviewed. The Gazebo outdoor play area was remeasured for capacity.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of staff records and interview, the center failed to obtain all required out-of-state background checks in the required timeframe.

Evidence:
1. Staff #1's date of hire was 12/20/2023. The sworn disclosure statement on file signed 12/20/2023 indicated they had lived out-of-state of Virginia within the past five years.
2. There was not an out-of-state criminal history background check in the record. The background check was required before the first day of employment.
3. There was not an out-of-state central registry background check in the record. The background check was required to be sent out by 01/20/2024. There was no evidence that an out-of-state central registry background check has been requested.
4. Administration verified that new out-of-state background checks had not been completed for Staff #1.
Plan of Correction: The out-of-state criminal history check was submitted on 02/23/2024 and received on 02/23/2024.The out-of-state central registry was submitted on 02/23/2024 and received on 02/24/2024. Moving forward, any background checks requested by any other institution will not be accepted. All new checks will be initiated upon hire through Heritage.

Based on review of staff records and interview, the center failed to ensure that a central registry finding was completed within 30 days of employment.

Evidence:
1. Staff #1's date of employment was 12/20/2023. There was no documentation in the record that the central registry finding had been requested.
2. Staff #3 verified that there was not a central registry finding in the record.
