Inspector Notes
An unannounced inspection was initiated on February 18, 2026 and completed on March 2, 2026 in response to complaints received by the licensing office on January 30, January 31 and February 11, 2026 relating to allegations of administration, staff qualifications and training, physical plant, staffing and supervision, programs, special care provisions and emergencies.
The inspector reviewed one staff record on-site. This inspection included document review, tour of the facility, and interviews. The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
