Inspector Notes
A monitoring inspection was conducted on October 4th as a result of a self-reported incident that was received by the licensing inspector on October 1, 2021. An in-person visit was made to the center on 10/4/2021. Center video footage was reviewed, 2 staff were interviewed and 1 children's record was reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on staff interview and review of center video one staff member failed to follow constructive behavior guidance for one child.
1. Center video was reviewed for an incident that occurred on September 30, 2021.
2. Staff A was observed picking up one child and placing them in a chair away from the group and not redirecting.
