Inspector Notes

An unannounced, on-site monitoring inspection was initiated on December 17, 2024 and completed on December 17, 2024. The on-site inspection began at 9:20 a.m. and ended at 12:25 p.m. The inspector reviewed compliance in the areas listed above. There were 22 children present and thirteen staff. The inspector reviewed six children?s records and six staff records on-site. This inspection included document review, tour of the facility, observations, and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence that contains staff and parent signatures or two staff signature.
An injury report dated 12/13/2024 only contained one staff signature.
Plan of Correction: A second signature was added to the injury report. Staff will be informed to ensure injury reports have taff and parent signatures or two staff signatures.
Standard 8VAC20-780-560-G
When food is brought from home, the food container shall be sealed and clearly dated and labeled in a way that identifies the owner.
In the 2's room a sippy cup of milk was in the refrigerator that was not dated and labeled.
In the transition room, four cups in the refrigerator were not dated.
Plan of Correction: Labels with dates were added to each cup. Staff were reminded to date all cups.
