During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on September 24, 2024. The on-site inspection began at 2:55pm and ended at 5:20pm. The inspector reviewed compliance in the areas listed above. There were 43 children present and four staff. The inspector reviewed four children?s records on site and four staff records electronically 9/27/24. This inspection included document review, a tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/11/24. A POC submitted after this date will not appear on the public website.

The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.
The current license was not posted on the licensed premises. The license that was posted had an expiration date of September 2023.

Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
There was a bottle of sanitizer in an unlocked cabinet accessible to children. The manufacturer?s label stated, ?Keep Out of Reach of Children.?

The required first aid kits shall include at a minimum all the required supplies.
The first aid kit available for review did not include gloves, as required.

The following nonmedical emergency supplies shall be required: one working, battery-operated flashlight on each floor and one working, battery-operated radio in each building.
The flashlight and radio available for review did not contain batteries and therefore, were not working. There were no batteries at the center for the emergency supplies.

Repeat Violation
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence.
Staff stated that there was a minor injury that occurred in September 2024. However, there was no documentation of the injury at the center for review.
